Online Health Appraisal Questionaire (HAQ)
To complete your health appraisal questionnaire online, please follow the steps below.
1 Go to www.metagenics.com.au
2 Click on the "Complete HAQ" hyperlink option in dark blue, along top of screen 3 Where it asks for the "Practitioner Member Number" enter - 39923
4 Enter full name, date and gender where asked
5 Proceed to HAQ. Complete as instructed and submit.
